PREMIUMS ARE CHANGING FOR ALL PLANS

Employees are allowed to make a change to their health plan during the open enrollment period, scheduled for May 20, 2024 – June 30, 2024. All changes will be effective October 1, 2024.
Changes You Can Make During Open Enrollment
- Medical Plan Changes
- Changing medical plan ⇒ complete the Plan Election Form
- Changing carriers (Anthem to Kaiser or Kaiser to Anthem) ⇒ complete the following three forms:
- Enrollment Form (Kaiser or Anthem),
- If moving to an Anthem HMO plan, you will need to look up your preferred HMO Primary Care Physician's ID number on Anthem’s website: anthem.com/ca/sisc (Find Care) and include this physician’s ID on the Enrollment Form; otherwise, Anthem will auto-assign a primary care physician for you.
- SISC Membership Change Form
- Plan Election Form
- Enrollment Form (Kaiser or Anthem),
- Add or drop dependents ⇒ complete a SISC Membership Change Form.
- If adding dependents, please see the Dependent Eligibility Documentation Checklist for required documents that must be submitted with your SISC Membership Change Form.
- Opting out of Medical Benefits ⇒ If you are choosing to opt out of medical benefits with us, you need to fill out this form. You will also need to fill out the SISC Enrollment form. In addition, you need to provide proof of coverage in order to decline benefits with us like a copy of your insurance card.
Important Dates
- For Open Enrollment, insurance forms must be completed and returned to the Benefits Department beginning May 20, 2024 through June 30, 2024 with all required documents.
- As a reminder, you can only make a plan change or add or terminate dependents during the annual open enrollment unless you have a mid-year qualifying event such as marriage, divorce, birth, etc., reported to the benefits office within 31 days of the event.
- Dental and vision enrollment changes are effective January 1 and open enrollment for those products is at a later date.
