MEDICAL OPEN ENROLLMENT: 06/01/2025 - 08/31/2025
PREMIUMS ARE INCREASING FOR ALL PLANS

Employees are allowed to make a change to their health plan during the open enrollment period, scheduled for June 1, 2025 - August 31, 2025. All changes will be effective October 1, 2025.
Changes You Can Make During Open Enrollment
Change Medical Plans
- Complete the Plan Election Form.
- If your plan switch involves switching carriers, please also complete the Enrollment Form for the carrier you are switching to: Kaiser or Anthem.
- If moving to an Anthem HMO plan, you will need to look up your preferred HMO Primary Care Physician's ID number on Anthem’s website: anthem.com/ca/sisc (Find Care) and include this physician’s ID on the Anthem Enrollment Form; otherwise, Anthem will auto-assign a primary care physician for you.
- Drop off completed paperwork at the District Office or Pony from the school site's intradistrict mail, Attn: Benefits/Kendel Browning.
Add or Drop Dependents
- Complete a SISC Membership Change Form.
- If adding dependents, please see the Dependent Eligibility Documentation Checklist for required documents that must be submitted with your SISC Membership Change Form.
- Drop off completed paperwork at the District Office or Pony from the school site's intradistrict mail, Attn: Benefits/Kendel Browning. Dependent documents can be copied and submitted.
Opt Out of Medical Benefits
To decline medical coverage under a District plan because you already have existing coverage which you prefer:
- Fill out the SISC Enrollment form.
- Fill out this waiver form.
- Provide proof of coverage. This is generally a copy of your current insurance card.
- Drop off completed paperwork at the District Office or Pony from the school site's intradistrict mail, Attn: Benefits/Kendel Browning.
Important Reminder
You can only make plan changes or add/terminate dependents during the annual open enrollment period, unless you have a qualifying event reported to the Cambrian Benefits Office within 31 days of the event (such as marriage, divorce, addition of child through birth/adoption, etc.)
