Employee-Initiated forms are available through PowerSchool Employee Records ("PER") and InformedK12 ("IK12"). We will be expanding the forms available to you in PER.

Currently, you are able to initiate the following forms:

Payroll Changes

  • Affidavit of Designation to Receive Warrants (who receives your pay if you die)
  • Deferred Payment Plan Option (adjust your decision to spread your pay over 10/11 months or over 12)
  • Direct Deposit Authorization
  • State DE-4 (state tax withholding form)
  • W-4 (federal tax withholding form)
  • Worker's Compensation - Pre-Designation

  • Emergency Contact and Medical Information
  • Employee Change of Address
  • Employee Legal Name Change Request
